EXEMPLARY PARTNERSHIP PROGRAM AWARD

Deadline for Submission: February 23, 2007

IMPORTANT NOTE: All entries must be submitted on a disk, along with six (6) hard copies. All disks must be labeled with the program name.

Follow these four steps to nominate a program for the Exemplary Program Award.

Step 1 | Step 2 | Step 3 | Step 4

Step 1 Describe your program in terms of the sections below and label each section of your description. You are limited to four pages of 8.5" x 11" paper, double-spaced, single-sided.

  Area l: Goals: How does this program support student achievement through its alignment with the New Jersey Core Curriculum Content Standards? State relevant program goal(s) and how it relates to the Core Curriculum Content Standard(s).

  Area 2: Activities: What are the activities and unique features of the program? (Include the number of volunteers, volunteer hours, students, parents, teachers, and corporate or community involvement).

  Area 3: Outside Resources: What outside resources (financial and human) support the partnership program? If your program is selected, how will you use the $1,000 to support your partnership program?

  Area 4: Evaluation: What has been the impact of the partnership program on the students, teachers, and community being served? Factors to be considered include the number of students involved, increased student achievement, increased parental involvement, and attitude changes of students and staff through pre- and post-test data.

  Area 5: Stability: Are the major organizational components of the program in place? Describe the stability of the program as a whole, as evidenced by the length of time in existence, status of the coordinator, sources of funding for program operation, recruiting and recognition of program participants, orientation and training.

  Area 6: Endorsements: Which major stakeholders in the school-community partnership support the program? Include brief statements by three individuals involved with the program (must be included in the four pages of narrative).

Step 2 Complete the information form.

Step 3 Make six copies of the information you produced in Step l. Make six copies of the form you completed in Step 2 and use them as cover sheets. Please submit printed copies along with the cover sheet and program description on an IBM/PC formatted disk or CD. Be sure to label your disk or CD. Winning entries will be listed on the NJAPIE Website. DO NOT SUBMIT ANY OTHER MATERIALS.

Step 4 MAIL THE DISK, ORIGINAL AND ALL COPIES TO:

DEADLINE: Entries must be postmarked no later than April 11, 2008.

All submissions will be notified in April.

Note: To qualify for this award, you or your organization must be a member of the New Jersey Association of Partners in Education. Click on Become a Member for a membership application.
